Completing and Submitting your Template

• Run the ME Report located in HR > Reports > Export
• Copy and paste the data into the template below and submit a request to CCT/CSM 
• Follow the guidance in this article on how to submit your completed template and what happens next

Once the updated template is passed to Fourth's data consultants these templates have a TEN (10) day lead time.
