The COVID Health Survey provides a simple tool to help Fourth's customers ensure that their shifts are staffed with healthy employees, reducing the risk of COVID-19 infection amongst their workforce and customers. Basic health information from employees is recorded and summarised to simple statuses, which helps managers to assess if they are safe to carry out their shifts or not.

Above Store Configuration
Above Store Users will soon be able to turn the Health Survey On and Off for all locations within their company. There is also an option to allow store-level control that will allow store level Managers to control the status of the survey per location.
Please note: the store-level control is still in production and will be released soon, follow our Release Notes page to stay up to date on the latest releases!
To Control the status of the Health Survey, navigate to the Above Store Console and locate the Health & Safety tab.
From here you will be able to select the settings of the Survey.
Site Level Configuration
When the setting is set to Default on the Above Store Console for the Health Survey. You will be able to turn on or off the survey from the Store settings page. You will need to be configured with the Edit Company Settings permission. Just navigate to the Store settings page and select Edit General Information. Then turn ON/OFF the survey and select save.
If the Health Survey was configured at the Above Store Console, you will see the following message.
