Health Survey Updates

What's Changing?
The Health Survey has been updated to include Food-Borne Illness questions.
Customers currently using the Health Survey will now see an update to several of the questions.
Question1:
Have you or someone you live with or take care of been diagnosed with any of the following diseases in the last 48 hours: E.coli, Shigella, Salmonella, Norovirus, Hepatitis A or COVID-19?
Question 2:
Have you experienced any of the following symptoms within the last 24 hours from an infectious condition?
- Fever over 100.4°F (38°C)
- A new or changes cough
- Difficulty breathing
- Fatigue
- Muscle aches
- Headaches
- Loss of taste or smell
- Sore throat
- Congestion
- Runny nose
- Vomiting
- Diarrhea
- Jaundice
- Open sore or wound
Question 3:
In the last 5 days, have you had close contact with someone diagnosed or suspected to have COVID- 19, including someone you live with?
Close contact is defined as physical contact OR being within 6 feet of an infected person for a cumulative total of 15 minutes or more over a 24-hour period.
Reason for the Change
Adding Food-Borne illness questions to the health survey will help slow the spread of infectious diseases to staff and customer. As with previous versions of the survey, any YES response will trigger a "may not be safe for work" notification to managers with access to the Scheduler.
Future updates to the survey are forth coming and the survey will be simply titled Health Survey (removing COVID-19 from the title).
