Required Follow Up Modals for Checkbox Tasks

The 4.0.64.0 Release (12.21.2017) introduced required Follow Up modals for out-of-tolerance Single Select Tasks.  Now, the same modals will be displayed for out-of-tolerance Checkbox Tasks which can be configured by administrators in the Above Store Console.
