Audit Trails and User Activity Tracking Report

We would like to have a path of report extract that shows audit trails.

1. Scheduling changes: We are looking to see what an employee (for example, HR Manager) has been up to on Fourth. Has she changed anything? Has she created anything? Has she approved any of the schedules? 

2.  Employee record changes: Who added/created a user? Who made changes on the employee record on Fourth?
