FTE days field in a report

I want to pull a report to see each e'ee, the FTE days field (from the employment screen),  their holiday entitlement and a flag to see if this is a calc or has been overwritten. eg full entitlement is 28 days if FTE days = 5; FTE days = 4 should give 22.5 days entitlement etc
