We're looking to improve our return to work interview process. Currently managers have to complete a return to work form which is manually scanned to Fourth as a document. We would like to set up a form which is very similar to the "Record new absence" form, only detailing the return to work interview. Is it possible to set up a new form to do this?
Currently there is only a "return to work completed' tick box on the absence form so the new template would provide additional details which could then be reported on each week to ensure that return to work interviews are being completed and that relevant information is being discussed at this meeting.
Is it possible to set up a return to work interview form on Fourth which is very similar to the format of the "add new absence" form?

There is not the ability to create bespoke forms within the portal, but if the uploading of a document is unsuitable for your needs a better way to manage this information might be with comments.
You could create a new comment category called 'return to work completed' from Comment Categories within the Administration drop down in HR. Although this would have to be created as a separate process to the absence management, it would sit in that employees file and could be reported on in Reports- Employee Comments and filter to your new comment category- Return to Work Completed.
