Hi @Jo Bolding​ 

The below article explains how to set up a Document type and how to upload the document in the record.

I would recommend creating a document type called 'Proof of eligability' for passports, birth certificates etc so that reports is clearer.

You can create a custom report to see which documents have been loaded.
